    The Individual Health Card is the document, issued by the Consejería of Health, which identifies citizens as users of the Public Health System of the Community of Madrid under equal conditions and facilitates access to its health benefits.

    The Community of Madrid Health Cards DO NOT HAVE AN EXPIRATION, as of January 1, 2017. Cards issued previously are VALID FOR ALL PURPOSES, despite the fact that the validity date stated on the card has passed.

    When their holders are foreign citizens, the validity of the health card will be adapted to the validity of the Foreigner's Identity Card or to the one that appears in the Citizen Registration Certificate of the European Union. Once the validity period of the health card has elapsed, you must request its renewal, providing the documentation indicated in the application form (see the Processing section).

Requirements

The Individual Health Card is issued to citizens residing in the Community of Madrid and whose right to health care is recognized by the National Social Security Institute (INSS).

Through this procedure you can request a health card or update the personal or address information of the current card.

Documents to submit

  1. Application. Press the PROCESS button to access the processing space where you can find the form.
  2. Documentation contained in the form. The documents that you must provide and the data that the Community of Madrid can consult are indicated.
  3. To update personal data at the request of people covered by Law 2/2016, of March 29, of the Community of Madrid, the signed Annex I must be provided (Process button at the beginning of this file to access information).

Remember that you are responsible for the veracity of the documents you present.

Applications submmission

The processing of this procedure can be done electronically or in person. If you choose electronic filing, you need to have one of the electronic signature systems recognized by the Community of Madrid.

To submit the application, click PROCESS, access the processing space and follow these steps:

  1. Prepare the documentation and/or annexes that you are going to provide with the application.
  2. Press DOWNLOAD and fill in the form. When finished, select the Save option.
  3. Access the electronic register for its presentation together with the rest of the documents.
  4. You can also present it in person at the authorized places for it, unless you are forced to interact electronically with the Administration.
  5. In case of face-to-face presentation, remember to ask if it is necessary to request appointment at the registration and citizen service office that interests you.

If you select electronic notification as the means of notification, the processing unit will send you the notifications through the Electronic Notification System of the Community of Madrid. To do this you must have an email address enabled in said service. To sign up, click on the access to the NOTE Service.

Once the request is registered, the service of file situation inquiry from where you can:

  • provide documents and send communications regarding your request and 
  • Check your processing status.

Procedure

  1. Citizens residing in the Community of Madrid who wish to request their Individual Health Card for the first time should go to Health Center that corresponds to you by reason of your domicile.
  2. Said request also can be done electronically (see the Processing section).
  3. Once the documentation submitted has been validated, the health card will be issued and sent to the HEALTH CENTER where the interested party submitted the application (Monday to Friday, from 8.30:20.30 a.m. to 3:4 p.m.), within an approximate period of 2-3 weeks. The interested party must confirm receipt of the card in advance, within a period of approximately XNUMX-XNUMX weeks.
  4. Until receipt of the health card by the interested party, for the purposes of the right to health care, the receipt of the request to obtain said card will be valid. Access to provisional healthcare will automatically cease if the right to public healthcare is denied by the competent body.
  5. If the interested party requires medical attention before receiving the requested card, they can go to their health center, where they will facilitate access to health care, providing proof of the request.

To consider:

  1. To access the health benefits and services of the Madrid Health System, the interested party must show the Individual Health Card, together with the holder's identity document, when required.
  2. Whenever you change your address, telephone number or other information provided for the issuance of your health card, you must notify it immediately:
    • in person, at your Health Center.
    • through this same telematic procedure, with a digital certificate or electronic DNI, through the option to modify personal data or address.
  3. This is especially important if the interested party has changed the condition that allows them access to assistance (for example, the change from beneficiary to holder or vice versa, the change from active worker to unemployed or pensioner, etc.).
  4. If you wish to change your doctor, go to your health center or use the online procedure established for this purpose on Free Choice.
  5. Observe that your address is correct in the option Print Available Data in the Population Information System of the Community of Madrid.
  6. You also have the possibility to consult/change your contact telephone numbers and email, if you have a digital certificate or electronic ID in the following link.
  7. The personal data collected for the issuance of the Individual Health Card will be incorporated and processed in the Population Information System and Health Card Management file, registered in the Personal Data Registry of the Spanish Data Protection Agency, and may not be transferred. , except for assignments provided for in the Law.
